THE CLARIFIER : Clinical Documentation Improvement Specialist 

The Role : 

  • Ensure records are complete before coding and defensible before submission.
  • Act as the final quality gate, validating accuracy, compliance, and audit readiness of every chart.
  • Identify and close documentation gaps, preventing incomplete records and protecting claim integrity.

What You Will Own – Your Responsibilities :

  • Documentation review — validating records against DME, Medicare, and payer standards using structured formats (SOAP)
  • Compliance verification — identifying missing, incomplete, or non-compliant documentation before Coding
  • Gap resolution — coordinating with clinical, coding, and EDI teams to close documentation gaps
  • Order & shipment tracking — tracking DME orders from confirmation to delivery and resolving mismatches
  • Audit support — conducting documentation audits and ensuring compliance readiness
  • Trend reporting — identifying recurring documentation issues and driving corrective actions

Non-Negotiable Metric : 

  • Documentation Accuracy Rate ≥ 98%
  • Percentage of records that pass compliance review without rework or escalation

Qualifications:

  • Experience:  1+ Years of experience in any field
  • Education: Any Graduate (Life Sciences preferred)
  • Knowledge: Basic understanding of medical documentation / compliance
  • Attention to Detail: High accuracy in reviewing records
  • Communication: Clear coordination and documentation skills
